The Plantrician Project (TPP), in partnership with Healthy Heart Nation (HHN), is excited to bring you the Health Equity and Lifestyle Project (HELP) conference. The conference is designed to explore the critical relationship between social determinants of health, health disparities and the role of evidence-based, lifestyle medicine as a critical and underutilized tool in addressing health disparities and reversing the chronic disease burden trend.
Understanding that where we live, work, play, pray and eat all play a role in positive health outcomes, the HELP conference will unite leaders from medicine, business, faith-based, government, and education to discuss the role of each sector in building the health of the community.
