Factory Farming and Public Health: Foodborne Illness and Its Wider Costs

Most food safety advice lives at the kitchen counter. Wash your produce, cook your poultry through, keep raw meat away from everything else. That guidance is sound, and it leaves out something important. A great deal of what decides whether our food makes us sick is set long before it reaches our kitchens, in how that food is raised and grown. In lifestyle medicine we spend a lot of time on what belongs on the plate, and this upstream part of the story deserves the same attention. It tends to get flattened into a simple plants good, meat bad message, and the real picture is more interesting and more useful than that. Some of what we assume about factory farming and foodborne illness holds up. Some of it does not. A plant-forward plate helps in specific, well-documented ways without being a guarantee of safety. And the story reaches beyond our own kitchens, to the workers, animals, and communities bound up in how this food is produced. Here is what the evidence shows, and what it means for how we eat.

Antibiotics are the clearest problem

The clearest connection runs through antibiotics. When large numbers of animals are raised close together, disease spreads easily, so the industry relies heavily on antibiotics, often not to treat sick animals but to prevent illness across entire herds and to promote faster growth. Around 70 percent of the world’s antimicrobials are used in food-producing animals.

That use does not stay on the farm. Resistant bacteria from poultry and livestock reach us through undercooked meat, milk, and eggs, and also through fruits and vegetables that pick them up from soil or water carrying animal waste, as the CDC describes. Each course of antibiotics acts as a kind of selection pressure. Susceptible bacteria die off, the resistant ones survive and multiply, and the genes that protect them move between species. Concern about exactly this pathway is why several European countries banned antibiotics for growth promotion. This is well-established science, and it is a large part of why antimicrobial resistance now ranks among the leading global health threats.

The lettuce problem

Fresh produce is responsible for a large share of foodborne illness, and leafy greens come up again and again in outbreaks of Shiga toxin producing E. coli. At first glance that can look like an argument against eating our vegetables. Look at where the contamination comes from, though, and the picture changes. Between 2009 and 2017, the FDA and CDC identified 28 STEC outbreaks linked to leafy greens, and investigation after investigation points to the growing environment, not the home kitchen, as the source.

The 2018 romaine outbreak is a clear example. It sickened 210 people across 36 states, sent 96 to the hospital, caused 27 cases of hemolytic uremic syndrome, and took five lives. When the FDA studied the Yuma growing region, it found the outbreak strain of E. coli O157:H7 in the canal water used to irrigate the romaine, water that ran alongside a large cattle feedlot. A separate outbreak the next year in the Salinas Valley turned up the same strain in soil and fecal material from a cattle grate within two miles of the fields. The lettuce did not produce the pathogen. It picked it up from the cattle operation nearby. When we talk about produce safety, we are often really talking about the footprint that animal agriculture leaves on the land and water around it.

Not every produce outbreak traces back to animals, though, and the most current example is a pointed reminder for anyone who eats a lot of vegetables. Through the summer of 2026, a large multistate outbreak of Cyclospora, a microscopic intestinal parasite, was linked to iceberg lettuce grown in central Mexico and distributed to retailers and restaurants, including some Taco Bell locations, which prompted a recall. As of early August, that lettuce-linked outbreak spanned nine states with 1,947 reported cases and at least 98 hospitalizations, and two people with underlying health conditions had died in Michigan. Nationally, cyclosporiasis was running well above its usual pace, with the CDC logging more than 6,700 laboratory-confirmed cases since May. Cyclospora is a different kind of problem from the E. coli story above. It spreads through food and water contaminated with human feces, not cattle waste, so the remedy is clean water and careful handling in the field and along the supply chain rather than a change in how animals are raised. It is a plain reminder that produce carries risks of its own, and that eating more plants is not the same as eating risk-free.

What leaving meat off the plate actually removes

This is where a plant-forward plate offers a real benefit, and it helps to be specific about what that benefit is. Federal agencies track which foods are behind our major foodborne pathogens, and the pattern is instructive.

Campylobacter is the clearest example. It causes more bacterial diarrheal illness than any other pathogen in the country, an estimated 1.5 million cases a year, and most people get it from poultry. The CDC notes that a single drop of juice from raw chicken can carry enough bacteria to make someone sick. A plate without poultry removes the main route to our single most common bacterial food infection.

The meat and fish borne parasites work the same way. Without pork, wild game, or raw fish, there is no Trichinella, no beef or pork tapeworm, no Anisakis, and a large share of Toxoplasma exposure disappears as well. Vibrio, which comes from raw shellfish, drops off the list too.

For Salmonella and E. coli O157, the benefit is partial, and it is worth being honest about that. Salmonella comes from a wide range of foods: chicken, pork, beef, and turkey, but also fruits, tomatoes and other seeded vegetables, and nuts. Setting animal foods aside removes a meaningful portion of Salmonella risk, not all of it. E. coli O157 leans even more toward produce, with more than 85 percent of illnesses traced to leafy greens and beef together, and greens the larger share. Cutting out beef helps. It does not finish the job.

In short, a whole-food plant-based way of eating takes several major foodborne pathogens off the plate entirely and reduces exposure to others. What it does not do is make us immune.

Widening the lens: the costs beyond our own plate

Foodborne illness is only one way factory farming touches human health. When we widen the lens, we see costs that never appear in an outbreak report and that fall hardest on people and animals with little say in the matter. As educators, I believe we owe our audiences the fuller picture.

Consider the workers first. Meatpacking is among the most dangerous jobs in the country. Human Rights Watch found that in 2017, meatpacking workers were nearly twice as likely to be injured, and more than fifteen times as likely to suffer a serious work-related illness, as the average manufacturing worker. Two studies commissioned by the USDA and released in early 2025 found that even at current line speeds, 81 percent of poultry-plant workers and 46 percent of hog-slaughter workers faced a significantly elevated risk of carpal tunnel and other upper-limb disorders. Much of this workforce is made up of immigrants, and fear of retaliation keeps many injuries off the books. The conditions are hazardous enough that in 2023 the Department of Labor found a sanitation contractor illegally employing 102 children, some as young as 13, on overnight shifts cleaning bone saws and other equipment with caustic chemicals across 13 plants in eight states.

The animals bear their own burden. Approximately 99% of U.S. farmed animals raised for meat, eggs, and dairy live in factory farms. Laying hens are commonly housed in battery cages that give each bird roughly the floor space of a sheet of paper, too little to spread her wings. Breeding sows spend much of their pregnancies in gestation crates barely larger than their bodies, unable to turn around, a setup associated with sores, infections, and repetitive stress behaviors. Broiler chickens are bred to grow so quickly that many develop leg and heart problems, and routine procedures such as beak trimming and tail docking are often performed without pain relief. These are largely design choices rather than necessities, and less crowded, better-managed systems reduce the harm. A growing number of states and companies have moved to ban gestation crates and battery cages outright. How much moral weight we give to animal suffering is a personal question, but the conditions themselves are well documented.

Finally, there are the neighbors. A single large operation generates enormous volumes of manure, often stored in open lagoons and sprayed onto nearby fields, releasing ammonia, hydrogen sulfide, and fine particulate matter into the surrounding air and water. These facilities are sited disproportionately in low-income communities and communities of color, a pattern studied most closely in eastern North Carolina. Residents living near hog operations report higher rates of respiratory and irritation symptoms, and research has linked that proximity to higher all-cause and infant mortality and to more emergency room visits and hospital admissions. This is the quieter cost of inexpensive meat, and it rarely appears on anyone’s grocery receipt. 

What the evidence supports

When it comes to foodborne illness, the case for relying less on factory-farmed animal products rests on two solid foundations: antimicrobial resistance, and the waste that industrial operations send into our water, soil, and crops. Widen the frame to include the people who do this work, the animals raised in confinement, and the communities living beside these operations, and the case only grows. Reducing demand eases all of these pressures at the source.

What the evidence does not support is the comforting idea that eating plants makes our kitchens safe on its own. Produce carries real risk, some of it seeded by the same animal-agriculture runoff we saw in the romaine outbreaks and some of it, as with Cyclospora, entirely its own, so washing, cooking when it matters, and clean hands and surfaces remain essential for all of us. A plant-forward plate is a powerful part of a larger, systems-level solution. It is not a personal shield, and our message is stronger and easier to trust when we say so plainly.
